Dr. John Falace is an in-network participant in most dental insurance programs. We also welcome patients who do not have dental insurance!

VISA and Mastercard and personal checks are accepted.

Limited customized payment plans may be offered on an individual basis.
Since oral appliances are used to treat a medical condition (sleep apnea), the procedure is considered to be medical rather than dental. For this reason, the procedure is covered by most medical insurance carriers. Dental insurance does not cover these appliances.

Treatment for simple snoring without sleep apnea is considered to be an elective procedure and is not covered by medical insurance.

Most medical insurance companies provide coverage for the diagnosis and treatment of obstructive sleep apnea. In many cases, this may include oral appliances. Although we are not in-network providers for any medical insurance companies, most provide some degree of out of network benfits. We encourage you to contact your insurerer to determine your specific benefits.

The fee for a mandibular advancement device will vary depending upon the type of appliance required and complexity of the situation. Payment is due at the time of service, and prior to the fabrication of an appliance. Payment may be made in the form of cash, check, or credit card. We will assist you in filing a claim for reimbursement from your insurance company, however, we cannot assume responsibility for those payments.
